Here’s What You Need To Know.

We work alongside GPs across South-West Sydney to deliver structured
allied health support under the Chronic Disease Management (CDM) program.

What we provide:

  • Clear, concise patient progress reports back to the referring GP after the initial assessment and at agreed intervals.
  • Programs aligned to the goals set in the GP Management Plan (GPMP) and Team Care Arrangement (TCA).
  • Coordinated care with specialists, dietitians, and other allied health providers.
  • Direct communication when a patient’s condition changes or further specialist review is warranted.

How to refer:

  • Patient must have a GPMP and TCA in place.
  • Referral on the standard EPC / CDM referral form.
  • Specify the number of services under the ‘Exercise Physiologist’ (Item Number 10953).
    Physiotherapy services are coming soon.

A Medicare-funded program for patients with chronic conditions expected to last 6 months or longer. It covers up to 5 allied health sessions per calendar year (across all allied health providers combined), once your GP has prepared a GP Management Plan (GPMP) and Team Care Arrangement (TCA).

For Medicare CDM rebates, yes — you need a current CDM referral from your GP. For private health, NDIS, DVA, or self-funded sessions, no referral is required.

Medicare provides a rebate per session which only covers part of the total fee. You must then pay the gap fee. Contact our team for more information before booking.

Most clients start weekly to build momentum and load capacity safely, then move to fortnightly or monthly as the program progresses. Frequency depends on your condition, goals, and funding.

Our rehab team takes a structured, long-term approach focused on progressive exercise as a primary tool for managing chronic conditions. Physiotherapy services are coming soon at Chronic Gains, meaning clients will soon have access to both services under one structured method, one team, one rehabilitation system.

Most private health funds with extras cover allied health sessions, including Exercise Physiology. Coverage varies — check with your fund. (Note: you cannot claim Medicare and private health for the same session.)
