Chronic Condition Management
Take Control Of Your Chronic Condition
Long-term conditions don’t go away, but they can be managed properly. Our rehab team in Bankstown, Sydney delivers exercise-based programs that improve symptoms, reduce risk, and rebuild capacity. Medicare CDM accepted.

You’ve Been Told To “Exercise More“.
That’s Not A Plan.

If you’re living with a chronic condition, you’ve probably been told to:
What you haven’t been given is a plan — built around your diagnosis, your medications, your current capacity, and what you actually want to do with your body again.
Structured, progressive exercise is one of the most effective tools for managing chronic conditions. It improves blood glucose control, lowers blood pressure, reduces joint pain, strengthens bone, lifts mood, and reduces all-cause mortality risk.
There’s a difference between doing some exercise and following a structured program designed around your condition. We do the second.
The current system is flawed.
It doesn’t work.

The Chronic Gains Method
3 Simple Steps To Chronic Condition Management
Most clinics treat symptoms. We rebuild how your body actually works.
Our structured 3-step rehab system is designed to create long-term results.
Assess Movement Deficits
We identify the real problem — not just where it hurts, but why it hurts.
This includes movement breakdowns, muscle imbalances, and weaknesses.
Rebuild Movement & Function
We correct how your body moves, targeting the injury and the areas above and below it to restore proper mechanics and support long-term recovery.
Structured Rehab & Performance Plan
We build a tailored program to progressively restore strength, control, and capacity — so you can return to work, sport, and life with confidence.

Injured my hip & groin, Doctors said to wait it out for 7+ weeks and prescribed pain meds however meds only help mask the pain. These guys did an in-depth assessment and developed a personalised rehab program. Saw major improvements in range of motion on the first visitation. Went from struggling to get out of bed and returning to combat sports in 4 weeks. Now feeling stronger than I was prior to injury.
I have been seeing An at Chronic Gains to help improve my thoracic and shoulder mobility. With An I had someone to guide me specifically on what I needed to do for my conditions and also provide accountability. Not only that, he was able to identify other areas for improvement which has been a real eye opener in terms of the difference between someone who provides a generic type of treatment vs a completely personalised one.
After about 4 months of physio [for shoulder and neck pain]… I tried to go back to the gym but I just felt as though something was not right. After that first session [at Chronic Gains] it was the most mind-blowing thing… finally feeling the right muscles engage. Having that ability where you are pain free… and being able to engage in your daily life without it being so detrimental.
