Billing & Rebates
Exercise Physiology may be funded by a range of Government Schemes and Private Health Funds including;
- Medicare Rebates on the CDM Plan
- NDIS (National Disability Insurance Scheme)
- Workers’ Compensation
- DVA (Department of Veterans’ Affairs)
- Most Private Health Funds
Medicare Rebates for Exercise Physiology sessions can be accessed under the Chronic Disease Management (CDM) Plan, formerly known as Enhanced Primary Care (EPC) Plan. This plan is designed to help patients manage their medical condition by providing access to allied health services, including Exercise Physiology.
Your doctor/GP will decide to put you on the CDM Plan if you have a chronic medical condition that has been present for at least six months, or is likely to be present for six months or more.
The CDM Plan provides Medicare Rebates on five sessions of Exercise Physiology per calendar year. The Medicare Rebate will cover a large portion and there will be a small gap.
You will require a referral from your doctor/GP for Exercise Physiology sessions, as part of the Chronic Disease Management Plan.

The aim of the NDIS is to provide support for individuals living with a disability to build skills and capability so that they can participate in the community, as well as in the workplace. The NDIS provides funding for allied health services related to a person’s disability – this includes Exercise Physiology.
Funding for Exercise Physiology can come from one of two budget areas in an NDIS plan:
- Capacity building – Improved Daily Living
- Capacity building – Improved Health and Wellbeing
Chronic Grains is an Accredited Exercise Physiologist (AEP) service provider to NDIS participants. This assures we are qualified allied health professionals with the knowledge and skills to design, deliver and evaluate effective exercise programs for people with disabilities.
We work with NDIS participants to improve overall health and well-being by designing programs that can make exercise safe and effective, promoting independence and improve quality of life for individuals with disabilities.

Workers Compensation is an insurance system set up to protect and rehabilitate workers who have been injured while at work. In NSW Workers Compensation is regulated by SIRA(State Insurance Regulatory Authority).
At Chronic Gains we are a SIRA approved Accredited Exercise Physiologists (AEP) enabling treatment for Workers Compensation and Motor Accidents Compulsory Third Party (CTP) insurance.
We work to rehabilitate injured workers through exercise programs that help improve strength, mobility, and function. Our programs are designed to help workers regain their physical abilities and return to work quickly and safely.
At Chronic Gains we work with Veterans to manage chronic health conditions and/or injuries with exercise rehabilitation. Our programs are designed to give Veterans the skills to effectively self-manage their health conditions through exercise.
For Veterans with a DVA White card, DVA will pay for Exercise Physiology sessions, if you require treatment because of an accepted war or service caused injury or disease.
DVA Gold cardholders are eligible to receive treatment for all their conditions.
You will require a referral from your GP, Medical Specialist or Health Professional as part of a hospital discharge.
Referrals will last up to 12 sessions or one year, whichever ends first.
Exercise Physiology services with the intent to prevent, delay or manage chronic health conditions or injuries are eligible for private health rebates.
Private health rebates are applicable if you have private health insurance with extras. Contact your health insurance provider if you are unsure of your health insurance cover.
Chronic Gains have a HICAPS machine that allows for claims to be made on the spot.
